Instead of buying and maintaining your own RPM system, you join a single shared, interoperable platform built and coordinated by Garnet for the benefit of all participating providers — reducing duplication, easing data sharing, and standardizing support across Vermont.
No cost to participate in the technology
RPM equipment, platforms, and training are grant-funded (100% CMS/HHS funded under RHT). Note funds cannot be used for clinical services already reimbursable by insurance.
Shared purchasing power
As the lead, Garnet negotiates platform and equipment terms on behalf of the whole consortium — better pricing than a single small practice could secure alone.
Less administrative burden
Garnet, as lead organization, manages the application, the budget workbook, SAM/UEI requirements, quarterly reporting, and compliance (HIPAA, 2 CFR 200, interoperability with Certified EHR Technology).
Stronger application
Applications scoring highest demonstrate multiple provider types working together and integration with other RHT initiatives (telehealth, e-consult, Blueprint community health teams). A consortium is built to score well here.
Hands-on support
Technology selection, workflow integration, cybersecurity and risk management, and staff plus patient training and onboarding.
Better rural access and outcomes
Earlier intervention for chronic conditions, fewer avoidable hospitalizations, and care that integrates into the patient's longitudinal health record.
Eligibility made simple
Open to home health (in-home and community), longitudinal care programs, long-term care, primary care/specialty, and community paramedicine practices physically located in Vermont. Rural practices are prioritized; urban practices qualify if at least 20% of unique patients reside in rural ZIP codes.

Express interest / commit

Practices sign up on this page and generate a Letter of Intent.

Garnet assembles the application

As lead organization, Garnet aggregates participating practices, sites, patient populations, and budget into a single statewide submission to Vermont AHS, proposing the unified RPM platform on behalf of all participants.

Submit by the state deadline

June 10, 2026.

Award & agreement

If selected, the grant agreement commences no later than August 1, 2026.

Platform build & onboarding

Garnet procures and stands up the unified statewide RPM platform and equipment, configures EHR interoperability, and trains staff and patients across participating practices.

Deliver care & monitor

Practices begin RPM with patients; data flows to clinical staff and the EHR.

Reporting

The consortium handles quarterly reporting (number of unique providers and unique patients using RPM; first report due October 30, 2026).
