Garnet is leading a consortium to build a unified, statewide Remote Patient Monitoring platform for Vermont providers.
Equipment, training, and support funded by the Rural Health Transformation grant — one shared statewide platform, built for the benefit of all participating providers.
Garnet is the lead organization on the application, coordinating a single, interoperable platform on behalf of every participating Vermont practice — rather than fragmented, practice-by-practice purchases.

Why join the Garnet-led consortium.
One shared platform, coordinated by Garnet for the benefit of every participating Vermont provider — less duplication, stronger application, real support.
An honest note: priority and scoring favor practices serving higher percentages of rural patients, so the form below asks about your rural patient share.
A coordinated, statewide process — from interest to care.
